REGISTRATION FORM:

Pre-registration is available on-line.

 

To pre-register or register and pay by credit card, please fill out the form below.

 

Fees must be received at least two weeks prior to class. Note that classes may fill up earlier than this deadline. Registration is not complete until payment has been received by the school.

 

To register:

*  Fill out the form below with name, address, etc.

*  Indicate the type of class you are interested in.

*  Indicate the desired class date.

*  Indicate your current certification if re-certifying

*  If re-certifying indicate your expiration date.

*  Be sure to indicate an email address if you have one.

 

If paying by check Indicate in the Comments section you are mailing a check, then print the form, then click Submit. Mail the printed form with your check.

 

If paying by credit card, be sure to fill out the registration form and submit it, then go to the Payment & Fees page to complete payment.
